Tee Box 360 – Liability Waiver and Release of Claims
By booking a simulator session, entering the facility, or using any equipment at Tee Box 360, the Participant agrees to the following:
1. Acknowledgment of Risk
Golf simulator activities involve risks including being struck by golf clubs, being struck by golf balls, ricochets from the impact screen, slips or falls, and equipment malfunction. These risks may result in injury, disability, or death.
2. Assumption of Risk
The Participant voluntarily assumes full responsibility for all risks of injury, death, or property damage while participating in activities at Tee Box 360.
3. Release of Liability
The Participant releases and discharges Tee Box 360, LLC, its owners, employees, and affiliates from any claims or liabilities for injury, loss, or damage arising from participation or use of the facility, except where prohibited by law.
4. Responsibility for Guests
Participants who book reservations for others are responsible for informing their guests of this waiver and accept responsibility for their conduct.
5. Equipment Use and Damage
Participants agree to use equipment responsibly and accept financial responsibility for any damage caused by themselves or their guests due to negligence or misuse.
6. Unsupervised Facility
Participants acknowledge the facility may operate without staff present and agree to follow all posted rules and supervise minors.
7. Medical Treatment
Participants authorize emergency medical care if needed and accept responsibility for related costs.
8. Indemnification
Participants agree to indemnify and hold harmless Tee Box 360 from any claims arising from their actions or use of the facility.
9. Video Surveillance
Participants acknowledge that video surveillance is used throughout the facility and consent to recording for security purposes.
10. Agreement
By signing electronically, booking a session, or entering the facility, the Participant acknowledges they have read and agree to this waiver and release of liability.
